CONTEXT

1 in 5 Irish people will experience a stroke in their lifetime, often with life-changing impacts.


StrokeLINK is a specialist nurse programme which provides post- hospital care to stroke patients. This programme started in 2019. It is currently being piloted in The Mater Hospital, Dublin, Ireland.

PROBLEM

The transition from hospital to home leaves many stroke survivors and their care partners without the clarity or confidence needed to manage symptoms. This gap in support increases the risk of readmission, placing additional pressure on already stretched acute hospital services.

INSIGHT

Accessibility improves the experience for people with vision impairments, including those affected by stroke, while also signalling an organisation’s commitment to diversity and inclusion. This helps make the website feel more trustworthy.

Solution

A 60+ page comprehensive and informative website. It is a tool that empowers users to take control of their recovery post discharge with its medical advice approved by medical professionals, ensuring confidence and decreasing chances of readmissions.
