StrokeLINK is a comprehensive service with multiple touchpoints designed to enhance patient care when discharged. Since the programme pilot, 98% of patients rate their post hospital care as excellent compared with 63% pre-StrokeLINK.

CONTEXT

1 in 5 Irish people will experience a stroke in their lifetime, often with life-changing impacts.


Many survivors feel afraid and unprepared when returning home, unsure where to find support. This lack of clarity can lead to poor engagement with medication and health advice, increasing the risk of further strokes - many of which are preventable with better self-care and clearer guidance.

CHALLENGE

How might we support and educate patients recovering at home to reduce readmissions and ease pressure on acute hospital services?

INSIGHT

Accessibility improves the experience for people with vision impairments, including those affected by stroke, while also signalling an organisation’s commitment to diversity and inclusion. This helps make the website feel more trustworthy.

OUTCOME

A 60+ page comprehensive and informative website. It is a tool that empowers users to take control of their recovery post discharge with its medical advice approved by medical professionals, ensuring confidence and decreasing chances of readmissions.
