Supporting stroke recovery beyond the hospital. StrokeLINK brings post-discharge care into the home, connecting stroke patients with the resources and community support they need to recover. Since its launch, “excellent” patient ratings have increased from 63% to 98%.
CONTEXT
1 in 5 people in Ireland will experience a stroke in their lifetime, often with life-changing consequences. Around 40% of stroke patients are readmitted to hospital within two years, adding pressure to an already stretched healthcare system.
StrokeLINK is a specialist nurse programme providing post-hospital support to stroke patients and their care partners. Established in 2019, it is currently being piloted at the Mater Hospital in Dublin.
PROBLEM
Recovery doesn’t end when patients leave hospital. At home, stroke survivors and their care partners can be left navigating recovery without the information, confidence or support they need.
StrokeLINK needed to find a better way to extend care beyond the hospital - helping people access the right support at home while reducing avoidable demand on acute services.
INSIGHT
Accessibility improves the experience for people with vision impairments, including those affected by stroke, while also signalling an organisation’s commitment to diversity and inclusion. This helps make the website feel more trustworthy.
SOLUTION
A 60+ page comprehensive and informative website. It is a tool that empowers users to take control of their recovery post discharge with its medical advice approved by medical professionals, ensuring confidence and decreasing chances of readmissions.
